ETNEP Program: Application


Prior to starting your application please refer to the application requirements found at http://caetacademy.ca/etnep-admission-standards/ It is the responsibility of the applicant to ensure that all elements of the application have been received by the CAET ETNEP office. Please contact the program administrator admin@caetacademy.ca with any questions regarding your application.


Please complete the following form to apply for the ETNEP Program . Ensure you have all your information readily available prior to starting the application. E.g. contact information of references.

Please have your references visit and fill out this form upon submission of your application.


The personal information collected on this form and any other personal information collected and maintained as part of a student’s records will be used for the purposes of admission, registration, and issuing receipts, graduation certificates and for CAET ETNEP research and planning. Contact information will also be disclosed to CAET for the purpose of providing membership information and to wound, ostomy and continence product manufacturers for the purpose of providing product information and samples. Personal information will not be shared with any other business or organization.


Proof of Criminal Screening

Although the CAET Academy does not require proof of Criminal Screening, some facilities in which students will complete their clinical preceptorships may require proof of a criminal screening. Students are advised to contact the Preceptor Coordinator regarding proof of criminal screening.

Refund Policy for ETNEP Program

The fee for attending the CAET Enterostomal Therapy Nursing Education Program (ETNEP) is $4500.00 CDN. Payable in three installments of $1500.00 prior to the start of each clinical course. The cost of the Professionalism course is included in these costs.

You may pay online for the ETNEP course with your VISA or Mastercard using the secure electronic payment processing form. Please complete the required information and click the "Proceed to Secure Payment Page" button only once when finished.

You will be emailed a receipt of the transaction for your records.

If you decide to cancel your payment during the payment process, you will be returned to this page.

 

Refunds before the program of study starts:

1. If written (email) notification of withdrawal is received by the CAET Academy Administrative Assistant more than fourteen (14) calendar days before the commencement of any course (professional Practice, Ostomy, Continence or Wound), the CAET Academy will retain $300.00 from the course tuition fees.

 

Refunds after the program of study starts:

1. If the CAET Academy Administrative Assistant receives written (email) notification of withdrawal or a student is dismissed, within Five (5) calendar days after commencement any course Professional Practice, Ostomy, Continence or Wound), the CAET Academy will retain $500.00 from the course tuition fees.

2. No refunds will be given after five (5) calendar days from the commencement of any (Professional Practice, Ostomy, Continence or Wound) course.

Preceptorship/Practicum:

Applicants must be prepared to complete a scheduled two-week (75 hour) practicum near the end of each of the three clinical courses. There are a limited number of qualified preceptors in some areas. Acceptance into the program requires that you be willing to travel to complete your practicum.

Preceptorship/Practicum:

I agree to abide by the rules and regulations for CAET ETNEP and as they may be amended from time to time. I certify that the information provided in my application materials is true and complete in all respects and that no relevant information has been withheld. I agree that CAET ETNEP retains the right to nullify my application if the information provided is false or incomplete. I certify that I am not under suspension or dismissal from a place of employment or academic institution nor have I been disciplined by my provincial nursing licensing body for reasons of professional or academic misconduct.

Physical Performance Checklist

The following are physical abilities needed to carry out the role/function of an ET Nursing Student. Check those, which you are able or not able to perform. If you require assistance or assistive devices in meeting these requirements, the ETNEP Director needs to be informed at the time of application.
